Sponsored Single Copy Copies purchased by a third party in quantities of 11 or more for distribution to consumers.
100% Single Copy Sales This publication reports no paid subscription circulation. Therefore, no average subscription price calculation is made.
Association: Deductible Copies served where the subscription was included in the dues of an association. The subscription was deductible from dues.
Association: Non-Deductible Copies served where the subscription was included in the dues of an association. The subscription was non-deductible from dues.
Partnership Subscriptions: Deductible Copies served where the subscription was included in products or services. The consumer could receive a rebate instead of the subscription.
Combination Subscriptions These are copies that are included in Paid Subscriptions Individual and were served to subscribers who purchased this publication in combination with one or more different publications.
Average Subscription Waiver for New Members The requirement to report an average subscription price has been temporarily waived due to the fact that the publication has not been an AAM member long enough to maintain the records required for calculating an average subscription price.
5% Subscription Circulation Less than 5% of this publication's circulation is individual subscriptions. Therefore, reporting of an average subscription price is not required.
Weighted Average Due to Frequency Change Due to a frequency change during the average price period, a weighted average frequency was used in calculating the Average Subscription Price.
Double Issues A double issue represents two copies of service. The Average Subscription Price Annualized is based on ____ issues, which includes ____ double issues.

Religious Publication Price Reporting Religious publications are not required to report an average subscription price. Prices utilized during the statement period are ____.
Congregation-Wide Subscriptions Copies that were served on yearly subscriptions that were paid for by parishes, mailed by the publisher to names and addresses furnished by the purchaser and, in some cases, delivered to recipient by the purchaser.
